ESSENCE STUDIOZ LLC — WAIVER OF LIABILITY AND RELEASE FORMApplies to all participants of Essence: THE COLLECTIVE Please read carefully before signing.

By signing below, I acknowledge that this waiver applies to the dancer registered above for Essence: THE COLLECTIVE, hosted by Essence Studioz LLC.

I understand that participation in this event involves physical activity that carries the risk of injury, illness, or other harm. I confirm that the dancer is in good physical condition and has medical clearance to participate in such activities.

In consideration of participation, I hereby release and hold harmless Essence Studioz LLC, its owners, directors, instructors, employees, agents, guest artists, and any staff on site, as well as Center City Mall of Paterson, from any and all claims, demands, liabilities, or causes of action arising out of injury, illness, property damage, or any loss incurred during the event.

I further agree to indemnify and defend all the above-mentioned parties from any claims brought by or on behalf of the participant or others related to the participant's involvement in this event, including any legal fees.

I understand that Essence Studioz LLC and its affiliates are not responsible for personal belongings that may be lost, damaged, or stolen on the premises.

I grant permission for photographs and videos taken during the event to be used by Essence Studioz LLC for marketing or promotional purposes, including websites and social media, without compensation.

I acknowledge that Essence Studioz LLC does not provide accident or medical insurance and that it is my responsibility to maintain adequate coverage. I accept full financial responsibility for any injury or health-related costs.

I understand that this is a pre-registered event and that all payments are non-refundable under any circumstances. No spots will be held without payment.

I have read and understand this waiver, and by signing below, I agree to its terms. This release is intended to be as broad and inclusive as permitted by New Jersey law. If any portion is deemed invalid, the remainder shall remain in effect.
